Select one choice for every day. The form will not submit if you have not done so. If you have taken a half day holiday, select 'Holiday' for that day. If you have taken toil, whether it be 1 hour or the whole day, select 'Work/Toil combo' and fill in the number of hours when prompted. Please enter 8 if you took toil the whole day in Summer, or 7.5 if you took toil the whole day in Winter.
This is an official record of your attendance. When you submit the form it is confirmation of your attendance. Falsifying this record is a criminal and disciplinary offence.
